Sensitive Data Access - Fred

About

The purpose of this form is for pharmacies to provide written consent for third party organisations (each a Third Party Organisation) to obtain access to, or for Fred to disclose, data and information about the pharmacy (Pharmacy Data), to a Third Party Organisation via the Fred software used by the pharmacy.

Fred IT Group works with Third Party Organisations who require access to Pharmacy Data held within Fred databases or who require access to real-time dispense and/or point-of-sale events. Fred provides credentials to these Third Party Organisations to access the required Pharmacy Data when authorised by the pharmacy to do so.

Requests for credentials and/or access to real-time events by Third Party Organisations are processed when:
  • An authorised representative of a Fred pharmacy grants its consent by the completion of this online form.
  • The Third Party Organisation has been provided by Fred with vendor credentials.
On receipt of this request, Fred will create and provide credentials directly to the relevant Third Party Organisation and/or provide notice to the Third Party Organisation of the pharmacy's authorisation. When a pharmacy requests credentials for internal use, Fred will provide the required credentials to the pharmacy. In both cases, credentials will only be created after the request has been validated by Fred.

A pharmacy and/or Third Party Organisation is required to advise Fred when these credentials are no longer required and/or are compromised.
Important Information
Before we begin, it’s important you note that Third Party Organisations are not controlled or operated by Fred. Therefore, the use and disclosure of the Pharmacy Data to a Third Party Organisation will be governed by the relevant Third Party Organisation’s terms and conditions and privacy policy. We suggest you familiarise yourselves with these before providing your consent later in this form.

The Pharmacy Data the Third Party Organisation/s will be given access to and be able to use may be de-identified information; personal information about you (for example pharmacy and pharmacist identifiers) and/or your patients (for example, their name, address and contact details); and also health and other sensitive information (for example information about your patients’ prescriptions, patient identifier (eg account number and Medicare details) and their medicinal preferences and/or customers).
